If you are not registered with a GP practice or would like to change your registered practice, you can obtain more information about GP surgeries in your area, the services they offer and opening times by calling your Patient Advice and Liaison Service (PALS) on [NUMBER].

You will also need to bring the following items along with your registration forms, either:
- photographic identification, such as a passport or driving license; and
- proof of address (this can be a letter that has your name and current address on and is dated within the last three months, such as a utility bill, bank statement, council tax letter or any other letter addressed to you).
In addition, if you are not a UK or EU citizen, we will need to see any relevant documentation from the Home Office as proof of your entitlement to access NHS services.
